A team from Planetree will visit our organization from

April 13– April 16, 2026

We invited the Planetree team here to evaluate how well this organization meets international standards for excellence in person-centered care. This is a way of delivering healthcare in ways that meets both your medical needs AND your human needs.

The best way for the team to understand how we are doing is to hear directly from you — our patients, family caregivers, community members, and staff.

During their visit, you may be approached by the Planetree team. If you feel comfortable, please take a few minutes to share your honest feedback. They are interested in hearing what we are doing well, as well as where you believe we can improve.

All comments will remain anonymous and will be included in a confidential report shared with the organization. Participation is optional.

Written Public Comment Period

March 30 – April 23, 2026

In addition to the site visit, we invite anyone who has had an experience with our organization to share feedback during a four-week public comment period.

To participate, please follow these guidelines:

• Comments must be submitted in writing.
• Please include the full name of the organization, as well as the city, state, and country.
• You may share feedback on any aspect of your care or work experience.
• All comments will remain confidential.
• Comments may be submitted online, by email, or by mail, and must be received no later than the final day of the public comment period.
